  • Applicant’s age on the effective date is used for rating.
  • Family includes the applicant, age 59 and under, the applicant’s spouse, age 59 and under, and dependent children at least 15 days of age and no more than 21 years of age. Premium is two times the eldest adult age 59 and under.
  • Maximum period of coverage: 365 days
  • Minimum premium: $20 per policy

Important Notes

  • This is not a replacement for a Canadian Government Health Insurance Plan. Please ask us about eligibility, what is not covered and the Pre-existing Conditions Exclusion.
  • In the event of a medical emergency, Alllianz Global Assistance must be notified within 24 hours of admission to hospital and before any surgery is performed.
    Limits on Coverage
    If you fail to notify Allianz Global Assistance without reasonable cause, then Allianz Global Assistance will pay 80% of the claim payable. You will be responsible for the remaining 20% of the claim payable.
  • It is a condition precedent to liability under your policy that at the time of application, you are in good health and know of no reason to seek medical attention.
